- Neuro-psychiatric Clearance (NP)

Requirements for LTOPF RENEWAL
Common Requirements to all applicants

- Drug Test clearance (DT)
- Neuro-psychiatric Clearance (NP)
- National Police Clearance
Step 1: FEO
Submit LTOPF Application form.
Verify FEO Online Account.
Step 2: Get your National Police Clearance
Please ensure that you have your PNP account prepared.
Step 3: Finance Service
Pay fees.
Notary - P200.00
Neuro Exam - P900.00 (P720.00 for senior with senior ID)
Drug Test - P300.00
National Police Clearance - P160.00
Step 4: Health Service
Take Neuro-Psychiatric Exam and Interview.
Results will be transmitted to your account.
Step 5: Crime Lab Group
Fill up form. Give urine sample.
Results will be transmitted to your account.
Step 6: Notary
Notarize form before uploading to FEO Online.
